PURPOSE: To establish
a standardized process to ensure that all patients are treated with
respect and dignity and to ensure patients are aware of their rights
and responsibilities with regard to their health care.
POLICY: It is the policy of Jeffrey
F. Klein, M.D., Inc. to maintain a mutually respectful relationship
with its patients. The practice makes clear its expectations regarding
the rights and responsibilities of patients to create a structure
for cooperation among patients, practitioners, the medical staff
and health plans.
PATIENTS RIGHTS AND RESPONSIBILITIES
- Patients will be provided with information about our services,
our providers and the patients rights and responsibilities.
- Patients will be treated with respect and recognition of their
dignity and right to privacy.
- Patients are encouraged to participate with their practitioners
in decision-making regarding their health care.
- Patients are encouraged to discuss appropriate and/or medically
necessary treatment options for their conditions, regardless of
cost or benefit coverage.
- Patients are encouraged to voice complaints or appeals about
the care theyre provided.
- Patients are informed of their responsibility to provide, to
the extent possible, information that their physician needs in
order to care for them.
- Patients are informed that they have a responsibility to follow
the plans and instructions for care they have agreed on with their
practitioners.
- The practice will provide information on patients rights
and responsibilities (see below) to all new patients and providers.
PATIENT RIGHTS:
- A patient shall have the right to access care without regard to
race, creed, sex, national origin, or religion.
- A patient should be treated with respect and dignity. He shall
have the right to considerate, respectful care at all times and
under all circumstances.
- A patient shall have the right to receive information from his
physician concerning his diagnosis, treatment and prognosis in terms
the patient can understand.
- A patient has the right to information from his physician that
is necessary for him/her to give informed consent to any procedure
or treatment.
- A patient has the right to refuse treatment.
- A patient has the right to a list of fees for services provided.
- A patient has the right to privacy and confidentiality.
- A patient has the right to expect reasonable safety insofar as
the medical office and environment are concerned.
PATIENT RESPONSIBILITIES:- A patient has the responsibility to provide, to the best of his/her
knowledge, accurate and complete information about present complaints,
past illnesses, hospitalizations, medications and all other matters
relating to his health.
- A patient has the responsibility to report unexpected changes in
his condition to his physician.
- A patient has the responsibility to comply with physician instructions.
- A patient is responsible for keeping appointments and when unable
to do so, notifying the medical office.
- A patient is responsible for his actions if he refuses treatment
or does not follow his physicians instructions.
- A patient is responsible for the financial obligations of his health
care.
- A patient is responsible for being considerate of the rights of
other patients and medical office personnel.
- A patient is responsible for being respectful of the property of
the medical office.
