DR. KLEIN'S POLICY REGARDING
THIRD-PARTY PHYSICALS AND FORMS
DMV and Pre-employment Physicals (includes vision and hearing
tests and urinalysis if required*)
Form fee included.
- $125 (many companies will reimburse the employee)?
If patient due for a health-screening visit that is reimbursable
by their insurance, the fee will only be $15 for the form, plus
the fee for any tests that are required to complete that form
that are not generally recommended for health screening for their
age.
Scouts/Camp/School/Sports Physicals (includes vision and hearing tests
and urinalysis if required*) Form fee included.
- $75 (pre-Kindergarten with state form is exception - N/C)?
If patient due for a health-screening visit that is reimbursable
by their insurance, the fee will only be $15 for the form, plus
the fee for any tests that are required to complete that form
that are not generally recommended for health screening for their
age.
Forms (without physical required, e.g. disability forms, some sports
releases, most day-care forms; if exam to be reported, generally must
have had complete exam in prior six months).
- $10-25 depending on complexity and time required for form.
Fees are to be paid at the time of visit. No billing for
third-party physicals or forms.
*Any drug screens and/or blood tests are extra.
